Fitness Facilities Use Agreement

Policy #427
I. Purpose
The purpose of this policy is to provide school district employees, and retirees in good standing, an understanding of the acceptable use of district fitness facilities.
II. Definitions
The fitness facilities are defined as the fitness center, Jag Gym, Small Gym, and Walking Track.
III. Agreement
By agreement, between the I.S.D. #2165 School Board and the district wellness committee, all district employees/retirees who have signed the use agreement and liability waiver, may use the fitness facilities only during the times allowed.
The superintendent may revoke use for any individual user at any time.
IV. Times of Acceptable Use
Daily 5 AM - 10 PM
High school students (athletes supervised by coaches or students supervised by teachers) may use the fitness facility during the school day or after school between 3:00- 4:30 PM on days that school is in session. Employees/ retirees and their guests may use the fitness facilities, when in compliance with the “Use Agreement”, at other times.
V. Acceptable Use
Only the Hinckley-Finlayson employees who have signed this agreement may use the key to enter high school and fitness facilities. The entrance must be used through the North East Doors of the Hinckley-Finlayson High School Office. All users are personally responsible for the care of the equipment and personal conduct. Those who do not care for the equipment and facilities at a high standard, behave unsafely or irresponsibly, will be denied the use of the fitness facilities.
District employees may have guests (12 years or older) use the facilities only if the guest is directly under the supervision of the employee. District employees/retirees must sign the liability waiver and use agreement in order to use the facilities. The liability waiver and use agreement will be filed and kept in the I.S.D. #2165 district office.
VI. Policy Review
The policy and agreement will be reviewed annually.
VIi. ID Badge
All employees/retirees who sign this agreement will be granted access via electronic ID Badge to use the North East Doors of the Hinckley-Finlayson High School Office. Any lost ID Badges must be reported immediately, replacement costs will be the responsibility of the employee. Any abuse or non-compliance with this agreement will result in revocation of access and privileges.
Use Agreement:
I, ______________________________, agree to keep the fitness facilities orderly and clean by doing the following: put weights, abdominal balls, and other fitness-related equipment away in the proper locations; clean the cardio machines after use by wiping down machines; keep stereo, CD’s, and other equipment from being removed from the fitness center. Shut off lights, television, and lock the doors when leaving the fitness area. I will accompany all my guests and take responsibility for their use of equipment.
and
I, ______________________________, agree to assume full responsibility and release Hinckley-Finlayson School District (I.S.D. #2165) from any liability arising from the use of any fitness facilities. I am individually, and fully responsible for any injuries resulting from use of the equipment or facilities.
